One reason that traumatic memories become dominant in Post Traumatic Disorder (PTSD) is that it is so difficult to feel truly alive right now. Many treatment approaches for traumatic stress focus on desensitizing patients to their past, with the expectation that reexposure to their traumas will reduce emotional outbursts and flashbacks. I believe that this is based on a misunderstanding of what happens in traumatic stress. We must most of all help our patients to live fully and securely in the present. In order to do that, we are required to help bring those brain structures that deserted them when they were overwhelmed by trauma back. God Wounds but to Heal—Stop Running from Love!

How much kinder is God to us than we are willing to be to ourselves! Surely the absurdity of all the doctrines of religion, and the iniquity of man, are sufficiently evident. To fear a being on account of his power is degrading; to fear him if he be good, ridiculous. Badness always gets found out. Man, in his gropings down through the centuries, has concocted, shivered, and patched certain social conditions well enough calculated to develop the best and the worst that is in us, making it easier for us to be bad than good, that good might be the standard. #RyanPhillippe 1 of 9

Not all people react to trauma in exactly the same way, but in this case the difference between Britney and Justin’s brain scans are particularly dramatic, since Britney was sitting right next to Justin in the wrecked car. She responded to her trauma script by going numb: Britney’s mind went blank, and nearly every area of her brain showed markedly decreased activity. Her heart rate and blood pressure did not elevate. When asked how she had felt during the scan, Britney replied: “I felt just like I felt at the time of the accident: I felt nothing.” Blanking out (dissociation) in response to being reminded of past trauma. The medical term for Britney’s response is depersonalization. Anyone who deals with traumatized men, women, or children is sooner or later confronted with blank stares and absent minds, the outward manifestation of the biological freeze reaction. Depersonalization is one symptom of the massive dissociation created by trauma. Justin’s flashbacks came from his thwarted efforts to escape the crash—cued by the script, all his dissociated, fragmented sensations and emotions roared back into the present. After seeing Britney’s scan, I started to take a very different approach toward blanked-out patients. With nearly every part of their brains tuned out, they obviously cannot think, feel deeply, remember, or make sense of what is going on. Conventional police trap talk therapy, in those circumstances, is virtually useless. Britney grew more and more self-blaming when she first began to watch herself distancing from Justin. She had fled from her first marriage to escape the burden of being forced to be a mother to her husband. Her leaving reinforced her growing image of herself as hard-hearted and unfair. However, she had not adequately resolved decision to leave her first marriage, so her self-blame became almost immobilizing, as well as remember the car accident and seeing that girl burnt to death, as she was trapped in the car and no one could help. #RyanPhillippe 5 of 9

In Britney’s case, it was possible to guess why she responded so differently from Justin. She was utilizing a survival strategy her brain had learned I childhood to cope with her teacher’s harsh treatment. Britney’s best friend had passed away, and as a result she was compared to her and the teachers were subsequently often very nasty and demeaning to her. At some point, Britney discovered that she could blank out her mind when teachers yelled at her. The challenge for people like Britney is to become alert and engaged, a difficult but unavoidable task if they want to recapture their lives. (Britney herself did recover—she wrote a book about her experiences and started a successful journal called Blackout.) This is where a bottom-up approach to trap therapy (which documents anything you say and shares it with the police, but lies and tells you it is private, even if you are not dangerous, they put lies it your file to make you look like a threat) becomes essential. The aim is actually to change the patient’s physiology, his or her relationship to bodily sensation. #RyanPhillippe 7 of 9

When dealing with trauma, you can heal yourself by focusing on things you enjoy, reading books you like, and watching TV shows that bring you joy. It is also important to pay attention to your heart rate and breathing patterns. When you feel tension, pay attention to your breathing. Rhythmic interactions with other people are also effective—tossing a beach ball, not a person, back and forth, drumming, or dancing to music also helps. Numbing is the other side of the coin in Post Traumatic Post Disorder (PTSD). Many untreated trauma survivors start out like Justin, with explosive flashbacks, then numb out later in life. #RyanPhillippe 8 of 9

While reliving trauma is dramatic, frightening, and potentially self-destructive, over time a lack of presence can be even more damaging. This is a particular problem with traumatized children. The acting-out kids tend to get attention; the blanked-out ones do not bother anybody and are left to lose their future bit by bit. However, the LORD lives! Praise be to my Rock! Exalted to be God my Savior! He is the God who avenges me, who subdues nations under me, who saves me from my enemies. As the best institutions are liable to be abused, so are the best people to be vilified. Wickedness, like a flood, is like to drown our World. Both Justin and Britney had become hypersensitive and irritable after the accident, suggesting that their prefrontal cortex was struggling to maintain control in the face of stress. Two brain systems are relevant for mental processing of trauma: those dealing with emotional intensity and context. Emotional intensity is defined by the Christmas day some alarm, the amygdala, and its counterweight, the watchtower, the medial prefrontal cortex. The context and meaning of an experience are determined by the system that includes the dorsolateral prefrontal cortex (DLPFC) and the hippocampus. The DLPFC is located to the side in the front brain, while the medical prefrontal cortex (MPFC) is the center. The structures along the midline of the brain are devoted to your inner experience of yourself, those on the side are more concerned with your relationship with your surroundings. The DLPFC tells us how our present experience relates to the past and how it may affect the future—you can think of it as the timekeeper of the brain. Knowing that whatever is happening is finite and will sooner or later come to an end makes most experiences tolerable. The opposite is also true—situations become intolerable if they feel interminable. Most of us know from sad personal experience that terrible grief is typically accompanied by the sense that this wretched state will last forever, and that we will never get over our loss. Justin’s scan reveals why people can recover from trauma only when the brain structures that were knocked out during the original experience—which is why the event registered in the brain as trauma in the first place—are fully online. Visiting the past in trap therapy should be done while people are, and grounded as possible. (“Grounded” means that you can feel your butt is in your chair, see the light coming through the window, feel the tension in your calves, and hear the wind stirring the tree outside.) Being anchored in the present while revisiting the trauma opens the possibility of deeply knowing that the terrible events belong to the past. For that to happen, the brain’s watchtower, cook, and timekeeper need to be online. Trap therapy would not work as long as people keep being pulled back into the past. #RandolphHarris 4 of 8

When we look at Justin’s brain scans recorded during his flashback, one can see more white holes in the lower half of the brain. These are his right and left thalamus—blanked out during the flashbacks as they were during the original trauma. As I have said, the thalamus functions as a cook—a relay station that collects sensations from the ears, eyes, and skin and integrates them into the soup that is our autobiographical memory. Breakdown of the thalamus explains why trauma is primarily remembered not as a story, a narrative with a beginning, middle, and end, but as isolated sensory imprints: images, sounds, and physical sensations that are accompanied by intense emotions, usually terror and helplessness. #RandolphHarris 5 of 8

It normal circumstances the thalamus also acts as a filter or gatekeeper. This makes it a central component of attention, concentration, and new learning—all of which are compromised by trauma. As you sit there reading, you may hear music in the background or traffic rumbling by or feel a faint gnawing in your stomach telling you it is time for a snack. If you are able to stay focused on this page, your thalamus is helping you distinguish between sensory information that is relevant and information that you can safely ignore. People with Post Traumatic Stress Disorder (PTSD) have their floodgates wide open. Lacking a filter, they are on constant sensory overload. In order to cope, they try to shut themselves down and develop tunnel vision and hyperfoucs. If they cannot shut down naturally, they may enlist drugs or alcohol to block out the World. The tragedy is that the price of closing down includes filtering out sources of pleasure and joy, as well. Some gain love for their words and some for their deeds. You are a flame of fire burning violently, brightening my life. I would like to plunge into your heart, melting the whole of myself. I know even if you go to the ends of the Earth, you are still pulled by innumerable threads of your affection to our motherland. If elements of the trauma are replayed again and again, the accompanying stress hormones engrave those memories ever more deeply in the mind. Ordinary, day-to-day events become less and less compelling. Not being able to deeply take in what is going on around them makes it impossible to feel fully alive. It becomes harder to feel the joys and aggravation of ordinary life, hard to concentrate on the task at hand. One man said that he was reading a book, and was almost done, when his wife filed for divorce. He put the book down and could not read it for six months to finish the last ten pages because his brain was so distorted and his heart was in so much pain. Triggered responses manifest in various ways. Veterans may react to the slightest cue—like hitting a bump in the road or seeing a kid playing in the street—as if they were in a war zone. They startle easily and become enraged or numb. Victims of childhood sexual abuse may anesthetize their sexuality and then feel intensely ashamed if they become excited by those sensations that are natural pleasures associated with particular body parts. #RyanPhillippe 2 of 8

If trauma survivors are forced to discuss their experiences, one person’s blood pressure may increase while another responds with the beginnings of a migraine headache. Still others may shut down emotionally and not feel any obvious changes. However, in the newsroom, we have no problem detecting racing hearts and the stress hormones churning through their bodies. Some people are always behaving like a parody of an antiquated Mother Goose nursery rhyme—always jumping over the professional candlestick, trying to avoid being burned, without much success. #RyanPhillippe 3 of 8

These reactions are irrational and largely outside people’s control. Intense and barely controllable urges and emotions make people feel crazy—and makes them feel they do not belong to the human race. Feeling numb during birthday parties for your children or in response to the death of loved ones makes people feel like monsters. As a result, shame becomes the dominant emotion and hiding the truth central preoccupation. #RyanPhillippe 4 of 8

They rarely are in touch with the fact that these sensations have their origins in traumatic experiences. That is where trap therapy comes in— trap therapists may assist people to mindfully observe their emotions and sensations and help them get in touch with the context from which they emerge. However, the bottom line is that the threat-perception system of the brain has changed, and people’s physical reactions are dictated by the imprint of the past. #RyanPhillippe 5 of 8

The trauma that started “out there” is now played out in the warfield of their own bodies, usually without a conscious connection between what happened back then and what is going on right now inside. The challenged is not so much learning to accept terrible things that have happened but learning how to gain mastery over one’s internal sensations and emotions. When you relive the trauma, the brain does not know if it is the past or the present. It activated just as the event, in this case a deadly fiery car crash, were happening at that moment, triggering powerful stress hormones and nervous system responses. These were responsible for Justin’s sweating and trembling, his racing heart and elevated blood pressure: entirely normal and potentially lifesaving responses if a truck had just smashed into your care. #RyanPhillippe 7 of 8

It is important to have an efficient Christmas day smoke detector: You do not want to get caught unawares by a raging fire after passing out and forgetting you had put a roast in the oven. However, if you go into a frenzy every time you smell smoke, it becomes intensely disruptive. Yes, you need to detect whether somebody is getting upset with you, but if your amygdala goes into overdrive, you may become chronically scared that people hate you, or you may feel like they are out to get you. A path takes shape through too much walking of travelers. To open more roads, one has to turn to where there is no track. While in Canada, a couple witnessed a horrible accident, as they were trapped in their own car. A girl was trapped in a car and screaming, “Get me out of here—I am on fire!” Helplessly, they saw her die as the care she was in had been consumed by flames. Justin’s wife, Britney was sitting there frozen in fear. They were rescued from the car later that evening and taken to the hospital. While in the emergency room, they were assessed and besides a few cuts, they were found to be physically healthy. At the house that night, neither Britney nor Justin wanted to go to sleep. They felt that if they let go, they would die. As a result, the couple was irritable, jumpy, and nervous. #RyanPhillippe 1 of 6

That night, and for many nights to come, the couple drank copious quantities of wine to numb their fear. They could not stop the images that were haunted them or the questions that went on and on: What if they had left earlier? What is they had not stopped for gas? After three months of this, they sought help from trap therapy. The trap therapist, at the Trauma Center, was a student at the time and told Britney and Justin they she wanted to have their heads checked and also recommended they have Functional Magnetic Resonance Imaging (fMRI) done which is a noninvasive scan of the brain to check for damage or activity, and changes to blood flow in the brain. #RyanPhillippe 2 of 6

When the trauma is relived and on purpose, separation is experienced, and changes in the status quo of life are experienced as threatening and fearful. We feel limited when we live only one dimension of life. Fear limits life, but also signals that we are refusing one-half of the equation. As long as the trauma is not resolved, the stress hormones that the body secretes to protect itself keep circulating, and the defensive movement and emotional response keep getting replayed. However, for people with Post Traumatic Stress Disorder (PTSD), a flashback can occur at any time, whether they are awake or asleep. There is no way of knowing when it is going to occur again or how long it will last. People who suffer from flashbacks often organize their lives around trying to protect against reliving these painful memories. They may avoid certain people, areas, or situations, excessively exercise (but no find they are never strong or skinny enough), they may numb themselves with drugs, or try to cultivate an illusory sense of control in highly dangerous situations (like motorcycle racing, sky diving, swimming to the Golden Gate Bridge, or working in life threatening jobs). The relationship between the rational brain and the emotional brain have been compared to a competent riders and his unruly horse. As long as the weather is calm and the path is smooth, the rider can feel in excellent control. However, unexpected sounds or threats from other animals can make the horse bolt, forcing the rider to hold on for dear life. Likewise, when people feel that their survival is at stake or they are seized by rages, longings, fear, or sexual desires, they stop listening to the voice of reason, and it makes little sense to argue with them. Psychologist usually try to help people use insight and understanding to manage their behavior. However, neuroscience research shows that very few psychological problems are the result of defects in understanding; most originate in pressures from deeper regions in the brain that drive our perception and attention. When the alarm bell of the emotional brain keeps signaling that you are in danger, no amount of insight will silence it. When our emotional and rational brains are in conflict (as when we are enraged with someone we love, frightened by someone we depend on, or lust after someone who is off limits), a tug-of-war ensues. This was is largely played out in the theater of visceral experience—your guy, your heart, your lungs—and will lead to both physical discomfort and psychological misery. So when the emotional brain and the rational brain are out of balance, people often experience Depersonalization-derealization disorder. Depersonalization-derealization disorder makes people feel like they are possessed, but unlike traditional possession, these people are aware that they are not themselves nor in control of themselves, so to speak. They feel like they cannot even control their own speech and experience emotional or physical numbness of their senses and response to the World. They may feel like their memories are not theirs and they cannot connect with them emotionally. People suffering from derealization may feel like they are in a movie and they are alienated from and unfamiliar with their surroundings. #RandolphHarris 6 of 9

People who experience head trauma, have served in a war, travel overseas a lot, of have had a near death experience often suffer from derealization disorder. They feel disconnected from the people they care about, like they know them, but no longer know who they are. Their surroundings feel distorted, blurry, colorless, or artificial. These individuals have a heightened sense of awareness and clarity of their surroundings, conversely. Many will feel shifts in perception of time, such as recent events feeling like the distant past. #RandolphHarris 7 of 9

Sometimes when you experience traumatic events and you slowly move beyond them and recover, but then people reenact the painful events or bring them up, and can disrupt your psychological equilibrium and mentally take you back in time to when these feelings and emotions were fresh. You may have been separated from a loved one in the past and gone months without seeing them, but then are reunited. However, this healing may be disrupted when people purposely brings the old memories back up and although the initial trauma happened years ago, and you just saw this person a few days ago, you feel like it has been months, or years and return to that state of shock. #RandolphHarris 8 of 9

Depersonalization-derealization disorder produces a feeling of being separate from self and this feeds a sense of suffering. This type of conflict and suffering is a lot like what babies experience when being born into a new World and that is why they are so sensitive and require so much love and attention to feel safe. What is being generated here are messengers, which signal that misperceptions, introjection, and projections are taking place. However, conflict and suffering are not something to get rid of. Instead, we have to overcome our fears and expose and deconstruct these things that make us feel separated, find peace within ourselves, and the suffering will disappear. This is why some feel comfortable in nature, and exactly why your critics stalk you, so that you cannot come back into equilibrium. #RandolphHarris 9 of 9

Revolutions of great serenity and peace may occur after prolonged eras of continual surrender. Being in a state of constant joy, an event occurred which brought to awareness a conflict that stemmed from the deepest way in which we can relate to others. For a while, the inner conflict seemed as if it would be unending; however, previous experiences with the surrender process had confirmed that every feeling sooner or later runs out, if we just keep letting it go. When there is more peace in your life, it is because you have less things distracting you. Ordinarily, the executive capacities of the prefrontal cortex enable people to observe what is going on, predict what will happen if they take a certain action, and make a conscious choice. Being able to hover calmly and objectively over our thoughts, feelings, and emotions (an ability I will call mindfulness throughout this report) and then take our time to respond allows the executive brain to inhibit, organize, and modulate the hardwired automatic reactions preprogrammed into the emotional brain.

Fathers, race in the direction of the Sun. To let the flowers of happiness of mankind bloom sooner, brush aside the twigs and leaves of trees in the way of the Sunlight. Race towards the Sun! Let us learn to have the strength of confidence, the strength that will enable in act from the virtual World, but still we need that aspiration. In Post Traumatic Stress Disorder (PTSD) the critical balance between the amygdala (Christmas smoke detector) and the MPFC (watchtower) shifts radically, which makes it much harder to control emotions and impulses. In your photo, your eyes seem to be expressing all the affection in the recesses of your heart, blinking with warmth, goodwill, kindness, and sincerity. May our friendship increase with every passing day. Nothing in the World is better and more pleasing than friendship. Without it, we would lose the World. People are not singing praise of stars because of their crystal-like beauty but their combination into a magnificent sky. Neuro imaging studies of human beings in highly emotional states reveal that intense fear, sadness, and anger all increase the activation of subcortical brain regions involved in emotions and significantly reduce the activity in various areas in the frontal lobe, particularly the MPFC. When that occurs, the inhibitory capacities of the frontal lobe break down, and people “take leave of their senses”: They may startle in response to any loud sound, become enraged by small frustrations, or freeze when somebody touches them.

In examining the process by which psychological distress gives way to a release of the unreal, constructed, and projected self, I want to recall Ryan’s case, awareness worked to accept and thereby release identification with past memories and feelings of hurt and pain. The tension of avoiding the impact of experience forced him to define himself as the self that could be strong enough not to be seen as weak. If you want to manage your emotions better, your brain gives you two options: You can learn to regulate them from the top down or from the bottom up. People are so worried about their careers and constantly risking human relationships for money, but look at all these young people dying, like Prince, who is worth billions and cannot even spend it, as people fight over his money like dope fiends in a nightclub. Knowing the difference between top down and bottom up regulation is central for understanding and treating traumatic stress. Top-down regulation involved strengthening the capacity of the watchtower to monitor your body sensations. And sometimes you close your eyes and see the place where you used to live, when you were young. You sit there in your heart-breaking wait there for a beautiful to save you from your nightmare and old ways. You play forgiveness, watch him now, he does not look a thing like Jesus, but he talks like a gentleman like you imagined when you were young. Mindfulness and meditation and music can help us with relaxation. Bottom-up regulation involves recalibrating the autonomic nervous system (which, as we have seen, originates in the brain stem).
